Position Summary: Allumé hand picks exceptional people committed to surrounding individuals and families with remarkable care. An Allumé Home Health Aide typically provides the following support to individuals in their home:· Helps clients in their daily personal tasks, such as bathing or dressing· Provides basic health-related services according to a client’s needs, such as checking vital signs or administering prescribed medication at scheduled times· Light housekeeping, such as laundry, washing dishes, and vacuuming in a client’s home· Organizes a client’s schedule and plan appointments· Arranges transportation to doctors’ offices or for other kinds of outings· Shops for groceries and prepare meals to a client’s dietary specifications· Provides companionship An Allumé Home Health Aide is assigned to specific clients by a Registered Nurse and performs mediation administration for clients with physician orders to administer medications and only upon delegation by the registered nurse. Qualifications:Successful completion of a formal State approved certification training program in medication administration resulting in certificationSuccessful completion of a written skills test and competency evaluation.Be at least eighteen (18) years of age.Minimum of six (6) months work experience in a supervised setting, preferably health care facility.Demonstrated ability to read, write, and follow a written Plan of Care.Good verbal, written, and interpersonal skills.Reliable means of transportation to/from all assignments. High school graduate (or equivalent) – preferred but not requiredRelevant experience with references.Car, driver’s license, auto insurance, and safe driving history.Ability to plan, organize, and multitask.Reliable, honest, and trustworthy.Willingness to participate in First aid training, Adult CPR and child and infant CPR, Allume Home Care Certifications. Essential Functions:Performs medication administration using a step-by-step procedure developed by the RN for each medication administered, each time.Performs simple procedures as an extension of therapy services under the direction and supervision of the therapist.a. Range of motion exercisesb. Assistance in ambulation or exercisesPerforms personal care activities, including but not limited to:a. Bathingb. Shampooingc. Skin care/nail cared. Oral hygienee. Shavingf. DressingPerforms household services essential to health care at home, including but not limited to:a. Meal preparation/feedingb. Laundryc. Light housekeepingd. Assists (by reminders and/or supervision, not direct administration) with medications that are ordinarily self-administered under the direction and supervision of the Registered Nurse (per state nurse practice laws and Agency policy)Reports any observed or reported changes in the client\'s condition and/or needs to the Registered Nurse or Therapist. Follows reporting guidelines in the Care Plan.Documents care provided, completes the forms required for the client\'s records. Completes the appropriate records to document care given and pertinent observations. Submits documentation per Agency requirements—at least weekly.Documents medications administered in the Medication Administration Record at the time that the medication is given and according to the standards for medication documentationPromotes personal safety and a safe environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager.Demonstrates safe practice in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs.Communicates effectively with all members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested.Maintains confidentiality in all aspects of the job.Successfully completes annual skills competency validation.Successfully maintains medication administration certification.Attends in-service programs to meet compliance requirements. Identifies learning needs to Agency supervisor.a.
