Duties Home-Based Primary Care (HBPC) is a home care program that provides comprehensive, interdisciplinary, primary care in the homes of veterans with complex medical, social, and behavioral conditions for whom routine clinic-based care is not effective. In contrast to other systems such as Medicare home care that target patients with short-term remediable needs and provide episodic, time-limited and focused skilled services, HBPC targets patients with complex, chronic, progressive disabling disease and provides comprehensive longitudinal home care. HBPC is designed to serve the chronically ill through the months and years before death, providing primary care, palliative care, rehabilitation, disease management and coordination of care services. HBPC targets primarily three types of patients in need of home care: * Longitudinal care patients with chronic complex medical, social, and behavioral conditions. * Longitudinal care patients who require palliative care for an advanced disease that is life limiting or refractory to disease modifying treatment. * Patients whose home care needs are expected to be of short duration or for a single problem not requiring an interdisciplinary team, when such services best help the facility meet the needs of this population. The Nurse Manager is accountable for the 24 hour management of a nursing clinical area. The Nurse Manager demonstrates leadership in delivering and improving holistic care through collaborative strategies with the Director of Nursing and others. The Nurse Manager is accountable for adhering to established policies, standards of care and standards of practice; demonstrating leadership in improving nursing care; and the administration, management, supervision and evaluation of both nursing care and nursing personnel of the designated clinical area. Home Based Primary Care has nursing staff based at the Seattle and American Lake divisions and the Bremerton, Port Angeles and Mount Vernon Community Based Outpatient Clinics. Qualifications **BASIC REQUIREMENTS:** * US citizenship. Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. * Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. * The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) * Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (ie, Puerto Rico) of the United States, or the District of Columbia. **Grade Determinations: **The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: * Nurse I Level I - An **Associate Degree (ADN) or Diploma in Nursing**, with no additional nursing practice/experience required. * Nurse I Level II - An **ADN or Diploma in Nursing** and approximately 1 year of nursing practice/experience; **OR** an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; **OR** a Bachelor?s of Science in Nursing (BSN) with no additional nursing practice/experience. * Nurse I Level III - An **ADN or Diploma in Nursing** and approximately 2-3 years of nursing practice/experience; **OR** an ADN or Diploma in Nursing and a Bachelor?s degree in a related field and approximately 1-2 years of nursing practice/experience; **OR** a BSN with approximately 1-2 years of nursing practice/experience; **OR**a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. * Nurse II - A **BSN** with approximately 2-3 years of nursing practice/experience; **OR** ADN or Diploma in Nursing and a Bachelor?s degree in a related field and approximately 2-3 year?s of nursing practice/experience; **OR** a Master's degree in nursing or related field with a BSN and approximately 1-2 year?s of nursing practice/experience; **OR**a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. * Nurse III - **Master's degree in nursing or related field** with BSN and approximately 2-3 year?s of nursing practice/experience; **OR** a Doctoral degree in nursing or related field and approximately 2-3 year?s of nursing practice/experience. **Preferred Experience:** * Master degree in Nursing or related field * National certification as Nurse Executive (NE-BC) or Nurse Manager and Leader (CNML) * Previous successful nursing practice in home care highly preferred * Knowledge of regulations and standards for home care Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (eg, Peace Corps, AmeriCorps) and other organizations (eg, professional; philanthropic; religions; spiritual; community; student; social). Education/Experience Requirements: * Bachelor of Science in Nursing * Minimum of 2-3 years successful nursing practice in medical/surgical or home care setting * Evidence of progressive leadership responsibilities **Physical Requirements:**This position involves considerable amount of standing (up to 2 hours), walking (up to 6 hours), stooping, bending, reaching, lifting, and turning. It requires repositioning patients, assisting with transfers from wheelchairs/stretchers and transporting them to other departments or facilities. Mechanical devices are available and are required to be used for heavy or difficult to lift clients. The work may involve pushing or lifting in excess of 45 pounds and going up and down flights of stairs. The position requires visual (corrected to 20/30 or better with color discrimination) and auditory acuity at a level to provide safe and effective nursing care. The position requires the nurse to work for periods of time with arms above shoulder level or with neck in a fully flexed or extended position. The nurse may occasionally be exposed to patients who are combative secondary to delirium, dementia or psychiatric disorders. The nurse many be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent must possess sound and logical clinical judgment. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. The position also requires ability to work on computers for extended periods of time.
