White Haven Center is located less than 2 miles off Route 80 at Exit 272 and sits on approximately 192 acres of land in Luzerne County. This picturesque campus (adjacent to the Poconos) with its rolling hills and nearby forest land offers a peaceful, private setting with the convenience of a downtown business area a short distance away. Our Center has been serving people with intellectual disabilities since 1956. As an Intermediate Care Facility for people with intellectual disabilties, the Center provides 24-hour supported living, medical care and developmental activities. An interdisciplinary team of staff provides a wide array of quality services to the adults who make White Haven Center their home.Currently, there are 163 men and women who call White Haven Center home, a home that serves as a multi-faceted residential facility for people with intellectual disabilities. It is a Center that has always been linked with leaders who were dedicated and energetic pioneers; people who set out to accomplish their dreams.PRIMARY CARE PHYSICIAN - White Haven CenterRole Description:This is professional medical work in the primary medical care and treatment of individuals with intellectual disabilities. The resource may serve as a primary care practitioner providing medical care and treatment services in a Commonwealth facility for individuals with a mental or physical injury or illness, dysfunctional behavior and/or an intellectual disability.Work includes responsibility for maintaining the continuity of the healthcare and medical treatments and guiding individuals and their interdisciplinary habilitative treatment team members in formulating and implementing comprehensive individualized care and treatment program plans.The physician provides clinical oversight in the delivery of medical care for individuals. Such oversight includes consultation and guidance of other health services staff including nursing. The physician will be expected to provide work related physicals for White Haven Center staff.Work is generally performed with limited or no direct professional supervision but is subject to review by the clinical and/or administrative supervisor through consultation, review of individual case histories, staff reports and personal inspection.Work week 37.5 hours. Estimate approximately 400 hours of on-call hours per year, if needed.Location address: White Haven Center, 827 Oley Valley Road, White Haven, PA 18661WORK STATEMENTProvide professional medical primary care to individuals residing at the White Haven Center. Responsible for providing medical leadership and direction to Interdisciplinary (ID) team.Duties and responsibilities include but not limited to:Admission notes and evaluations.Discharge Summaries.Direct and participate in ID Team functions which includes individuals serviced and their families and advocates in the decision making process to the degree appropriate to the individual's circumstances.Required legibly written documentation in the chart consistent with agency procedures, regulations, and professional medical standards.Maintain and upgrade skills by attending meetings, training sessions, and seminars and by reviewing current literature on clinical topics. Obtain fifty (50) or more hours of Category I CME training per year.Review of laboratory reports and test results related to medication and other aspects of medical management.Inform the ID Team of the expected benefits and attendant risks, including adverse drug responses and drug-to-drug interactions, of all recommended medication regimens.Confer with outside specialists/hospitals as medical needs dictate.Physician will be scheduled five days per week which consist of Monday through Friday. The physician will work 7.5 hours per day (est. hours 1950 per year), which will consist of a schedule of 8 .30 am to 4.30 pm excluding state holidays. The successful candidate will be required to sign in and out at the beginning and end of each shift worked. One-half hour will be deducted for lunch unless authorized by contract monitor.Provide on-call coverage as indicated above (see Role Description).Must possess Microsoft Office knowledge/skill.Hours are estimated and may be increased/ decreased as determined by assessment and evaluation. MINIMUM QUALIFICATIONSGraduation with a degree of doctor of medicine or osteopathy from a medical school approved or recognized by the Pennsylvania Medical Licensing Board or the Pennsylvania Osteopathic Licensing Board.Successful completion of an approved US residency training program; with emphasis in the allopathic medical specialties of family medicine, pediatrics, internal medicine, or certification in the osteopathic field of general practice.Every two years successful completion of 100 hours of continuing medical education approved by the American Medical Association or the American Osteopathic Association as required by the Pennsylvania Board of Medicine.Three years post-residency clinical practice experience with substantial emphasis on primary care.Possession of a current unrestricted licensure to practice medicine in Pennsylvania issued by the Pennsylvania Medical Licensing Board, or Osteopathic Medicine Licensing Board.Possession of a current unrestricted certificate to prescribe medication from the US Drug Enforcement Agency.Board certification or Board Eligibility in field of general practice.Successful candidate must have an active and eligible Medicare number and may not be excluded from participation in a federally funded healthcare program. The Medical and Clinical Service Directors will check for possible Medicare fraud; using the Federal Government website HHS-OIG Fraud Prevention & Detection ().Recent report from the National Practitioner Data Bank as determined acceptable by the White Haven Center's Medical Director (report within the last 12 months).Recent report from the Federation of State Medical Boards as determined acceptable by the White Haven Center's Medical Director (report within the last 12 months).Recent report from the American Medical Association as determined acceptable by the White Haven Center's Medical Director (report within the last 12 months).Demonstrated leadership experience/understanding of inter-disciplinary team approach to healthcare.Preferred vs. Demonstrated knowledge regarding the medical management of healthcare (including behavioral health) issues confronting a developmentally disabled population.The Primary Care Physician will be scheduled to provide occasional on-call coverage as needed, as determined by the contract monitor.
