PURPOSE/SUMMARY: The Admission nurse is a member of the interdisciplinary team who provides professional hospice care as prescribed by the physician in accordance with the policies and procedures of Yolo Hospice. The Admission nurse renders care that is within acceptable professional practice standards in compliance with the medical plan of care and state and federal guidelines. SUMMARY OF ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES Obtains referral information from Patient Access. Meets with patients and families to explain the hospice program of care and obtains appropriate signed consents from the patient and legal representative to begin hospice care. Completes an initial Nursing Assessment and Plan of Care, and obtains the order to admit to Yolo Hospice from the Medical Director or designee. Evaluates the immediate need for supplies, equipment, medications and services and coordinates the provision of these services. Completes an Admission Note, including a Trajectory of Decline and identifies the tip of the iceberg problems. Serves as a consultant and resource for the hospice staff to facilitate continuity of care and enhance the quality of care. Maintains NHPCO Standards of Care. Communicates family/patient and physician concerns to the team. Assumes responsibility for compliance with state, federal and local regulations in relation to admissions and practice. Accepts assigned duties in a cooperative manner. Attends monthly staff meetings, continuing education and in-service programs. Attends IDT meeting to facilitate communication of patient/family needs at admission Cross trains to all job responsibilities of Patient Access related to referrals and Admissions. Works cooperatively with patients, families, nursing staff, physicians, consultant personnel, and ancillary service providers. Follows established standards of practice and Yolo Hospice policies and procedures. Performs other duties as directed by the Associate Director of Patient Access, Director of Patient Care Services, or Executive Director. REQUIRED JOB QUALIFICATIONS California RN license required Graduate of an accredited school of nursing with BachelorA cents € (TM) s degree in nursing. Minimum of one year acute care/hospice experience preferred. Must be knowledgeable, at minimum, about Hospice Medicare Conditions of Participation and California Hospice Licensing Standards. Excellent communication skills, both verbal and written, are required. Public speaking experience is preferred. Ability to work effectively with community physicians and other health care providers (all disciplines) toward specified goals is required. Must be self-motivating individual who can work autonomously, set own priorities, organize work, take initiative and make independent decisions relating to job tasks. Skills is education, marketing and the development of formal educational materials are preferred. Must be computer literate in Microsoft Word. Power Point software is preferred. Ability to successfully meet agency background screening requirements. PHYSICIAL AND OTHER JOB SPECIFICATIONS Ability to communicate well in English (spoken and written) is required. Ability to communicate in Spanish is preferred. Able to use computer keyboard in order to perform job duties Able to lift and carry up to 15 pounds. Must be willing and able to flex hours to meet occasional appropriate business needs during evenings or weekends. If driving on Yolo Hospice business, must have a valid California driverA cents € (TM) s license and state-required automobile insurance.Source:
