ESSENTIAL DUTIES AND RESPONSIBILITIES
Know our residents and their family by spending time getting to know them.

Support our residents in adopting a successful aging lifestyle and celebrate their accomplishments and efforts.

Be passionate about the potential growth and confidence of our residents.

Receive and follow schedule/instructions from the Director of Nursing and as outlined in the established policies and procedures.

As directed by the Nurse Manager and with input from the resident, family, physician, and other associates, develop and revise, as needed, a service plan for each resident. Service plans are based on yearly or more frequent assessments of the resident s capabilities and needs and on resident care conferences organized, scheduled, and led by the nurse or others.

Carry out the nursing portion of the service plans including medication management duties:

1. Review medication profiles of residents and prospective residents.
2. Working with the consulting pharmacist, initiate medication management for new residents.
3. Monitor the medication program and procedures and develop improvements.
4. Order and reorder medications from the pharmacy.
5. Receive medication from the pharmacy.
6. Administer medication, assess response, and record actions.
7. Train and monitor non-licensed associates who supervise self-administered medications.
8. Carry out physician-ordered treatments.

Maintain clinical records of residents and help develop improvements in record-keeping

Communicate with residents, families, and the residents' physicians.
Communicate and follow-up with others regarding resident/family concerns.

Be aware of and coordinate services provided to residents by third parties including families, home health agencies, hospices, and private caregivers.

Maintain residents' rights to fair and equitable treatment, self determination, individuality, privacy, property and civil rights, including the right to make a complaint.

Assist with billing of residents for services provided by completing charge slips for nursing service and turning them in per the procedure. Assist with inquiries and concerns from residents and their families about services and charges.

Work with other nursing associates in a collegial manner to achieve good nursing process and favorable results for the residents.

Report all incidents/accidents, unsafe/hazardous conditions, equipment malfunctions or breakdowns per procedure.

Assist with establishing a program for providing healthcare related supplies to residents. These supplies may include medication boxes and cassettes, first aid supplies, gloves, assistive and adaptive devices, etc. Ensure that supplies are available in adequate amounts.

Understand and assist with training associates in the Fire Safety Program.

Respect the rights of residents and their families to fair and equitable treatment, self-determination, individuality, and privacy.

Greet new residents and families during move-in.

Assure that work/assignment areas are clean and equipment, tools, supplies, etc., are properly stored and maintained. Keep work area(s) free of hazardous conditions. Assure that all safety rules/regulations are followed at all times.

Attend and participate in associates meetings and follow up as needed.

Work together with fellow nurses to cover each other during vacations, sick leaves or other absences to ensure resident and family needs are met.

Develop and maintain a good working relationship with other associates and with the residents and their families.

Be involved in sales events (for example, open houses and tours) as they occur.
Other related duties as may be necessary or as directed.
QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. License: Must be licensed as an L.P.N. or R.N.
Experience: At least two years experience as an L.P.N. in a long-term care or hospital setting.SPECIFIC REQUIREMENTSMust be able to read, write, and speak English.

Possess the ability to make independent decisions when circumstances warrant such action.

Possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

Ability to prioritize and delegate tasks.

Be able to follow written and oral instructions.

Knowledgeable of safe practices, procedures and guidelines.

Ability to minimize waste of supplies, misuse of equipment, etc.

Must adhere to dress code as per community policy.PHYSICAL REQUIREMENTS The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.

Must be able to lift 30 pounds, push, pull and move equipment and supplies throughout the day as necessary. Required to share on-call duty for emergency conditions/situations.
