MENTAL HEALTH RESPITE MISSION STATEMENT: Through professionalism, nurturance, and empowerment MHR will provide supportive services with the purpose of leading participants in personal growth and improving life skills for transitioning to the least restrictive level of care within the community environment.SUMMARY: Provide intensive case management utilizing a strength:based, person:centered approach designed to promote recovery, promote meaningful participation in the community of choice, resolve crisis and promote stabilization within the community, and prevent need for Emergency Protective Custody, Mental Health Board Commitments, and other utilization of higher levels of care. Promote successful transition into the community. Willing to utilize trauma informed practices and have a philosophy that is client centered and recovery orientated.ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other similar and related duties may be assigned.Participant Services Respond rapidly to crisis situations including a thorough comprehensive risk assessment and safety plete comprehensive screenings such as Global Appraisal of Individual Needs (GAIN) screenings and Mental Illness and other assessments and interventions while in the community.Develop ISP within 3 days of admission in partnership with the consumer and support system that includes a crisis relapse/prevention plan and immediate supports and referrals.Meet weekly with program participant to monitor progress and barriers with ISP completion and discharge process and complete the following for participants as needed: provide program participant advocacy as needed; assist in obtaining benefits such as Social Security Insurance (SSI), housing vouchers, food stamps, Medicaid, etc; provide referrals to appropriate community:based behavioral health services; develop, activate and/or maintain formal and informal support systems; assist program participant in applying for appropriate financial, residential, mental health, and support resources in the community.Provide education groups to program participants and family or significant others as needed.Assist program participant on following their Wellness Recovery Action Plan (WRAP)municate with Clinical Reform Team (CRT) on referral and participation of CRT program participants.Monitor program participantsA? participation in all aspects of the municate pertinent information to therapists, psychiatrists, emergency contacts and team members at MHR.9. Will continue to utilize and refine trauma informed skills through training and practice.Programming Complete participants Self Responsibility plan within 3 days of initial contact.Review participantsA? file and assessments to complete Assessment Summary form which will provide direction on services needed for participant to successfully transfer out of services to least restrictive level of care.Collaborate closely with participant, family or significant other as determined by consumer, treatment providers, peer support, law enforcement and local emergency services.Review program issues and participantsA? progress, barriers and treatment recommendations weekly and as needed.Present program participants within two weeks of intake, at time of extension, and as need to Multi:Disciplinary Team to obtain feedback to address any barriers or clinical issues.Participate in on:call rotation providing guidance on staff and participant related issues in crisis, this may include covering shifts when there is lack of coverage.Assist in maintaining case records, compiling statistics and writing participant reports as requested by the Mental Health Respite Coordinator.Provide documen
