Under the Supervision of the Director of Community Mental Health, the Program Manager/Leader has clinical, administrative, and supervisory responsibility to the ACT team. This position is required to evaluate the multidisciplinary team in conjunction with appropriate psychiatric support to ensure service excellence and courteous, helpful, and respectful services to program clients; and function as a practicing clinician on the team.

The Cuyahoga County Assisted Outpatient Treatment program is designed to reduce the incidence and duration of inpatient psychiatric hospitalization, homelessness, and interaction with the criminal justice system among persons with Severe and Persistent Mental Illness (SPMI); and to improve the health and social outcomes of persons with SPMI. In order to improve outcomes for persons with SPMI, FrontLine utilizes the Assertive Community Treatment (ACT) model. The ACT model is a multidisciplinary team approach with assertive outreach in the community. The consistent, caring, person-centered relationships have a positive effect upon outcomes and quality of life. Below is a summary of the AOT program as well as strategies to improve client outcomes.

Qualifications: Masters degree in social work or counseling with an LISW or LPCC
